Release of Liability For Church Activity
Activity: Youth Camp
Dates: Friday July 29, 2022 - Sunday July 31, 2022
I, the parent(s)/guardian(s) hereby release and agree to hold Charisma Life Church harmless from, and waive on behalf of my child(ren) , my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the Church, or that may otherwise arise in any way in connection with Charisma Life Church or the Charisma Life Church Youth Camp.
I understand that this release discharges Charisma Life Church from any liability or claim that I, my heirs, or any personal representatives may have against the Church with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, this youth event. This liability waiver and release extends to the Church together with all board members, partners, and employees.
On the days of Friday, July 29, 2022 - Sunday, July 31, 2022, I acknowledge that I will be prompted and required to answer the following questions regarding COVID-19 (SARS-CoV-2)before participating in any event activities.
Must be able to answer YES to the following questions:
● I acknowledge my child(ren) is/are not experiencing any symptoms of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.
● I do not believe my child(ren) has been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19.
● I acknowledge my child(ren) has not been diagnosed with Coronavirus/Covid-19 and not yet cleared as non-contagious by state or local public health authorities.
● I acknowledge my child(ren) is/are following all CDC recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19.
I, the parent or guardian, hereby consent to the fact that my child(ren)’s picture may be taken throughout the week as they participate in different parts of day camp and that these pictures may be posted for social networking services and Charisma Life Church media reproduction.
